Why Peeing While Lifting Weights Happens and How to Stop It

Among 342 women surveyed across USA Weightlifting, USA Powerlifting, and CrossFit affiliates, 64% reported urine leakage and 44.8% reported leaking during training itself. Researchers at the Norwegian School of Sport Sciences and George Fox University published those numbers in a 2025 prevalence study of female strength athletes. Peeing while lifting weights is not a rare malfunction. It is a predictable response to pressure.
You are mid-set during a heavy barbell squat, or maybe hitting your stride a mile into a run, when it happens. A tiny drop, or perhaps a noticeable trickle. Bladder leakage during exercise is incredibly common. It happens to elite powerlifters, casual joggers, and moms returning to gym routines alike. Common does not mean permanent, and it does not mean you have to accept it as your new normal.
A widespread misconception holds that leaking means your body is broken or that you are using unsafe lifting techniques. Your body is not broken, and your technique is not inherently dangerous. Leaking signals that your body is navigating internal pressure in a way your muscles aren't quite ready to handle yet.
How Common Leaking Is Among Lifters and Runners
The clinical name for this is stress urinary incontinence (SUI), which the American College of Obstetricians and Gynecologists defines as leaking urine during effort, including walking, running, and exercise. When it shows up specifically during training, clinicians call it exercise induced urinary incontinence.
The same 2025 study found that Olympic weightlifters carried a higher risk of stress urinary incontinence than CrossFit athletes, and that leakage correlated with both parity and weekly training hours. The more you train, and the heavier you load, the more often the pressure system gets tested.
Participants also described something worth naming directly. Many had quietly decided that urine leakage was an acceptable side effect of training, and some had dropped specific exercises from their programming rather than address the cause. Embarrassment kept the problem in the dark. Silence is why so many strong athletes assume they are the only one in the gym dealing with it.
Understanding Pressure Spikes in Strength Training
To understand why you leak when you lift or run, we have to look at how your torso handles weight. Think of your midsection as a pressurized soda can. This is what we call the core canister relationship.
Your core is an enclosed system made of four main walls that work together to manage pressure:
The Top: Your diaphragm (your main breathing muscle)
The Sides and Front: Your abdominal muscles
The Back: Your deep back muscles
The Bottom: Your pelvic floor muscles
When you run or lift weights, your body naturally creates a rigid trunk to help you transfer power efficiently into the barbell or the ground. The mechanics of bracing involve squeezing these core muscles to build internal pressure, which creates a stable foundation for movement. When you run, every foot strike sends a wave of pressure through your torso. When you squat or deadlift, that internal pressure spikes even higher to help you lift the load.
If the pressure inside this canister becomes greater than the closing strength of your pelvic floor, the pressure pushes downward along the path of least resistance, and leakage occurs.

Which Lifts Provoke Leaking Most Often
Athletes in the 2025 survey named the same movements repeatedly. The pattern is consistent: leaking clusters around lifts and drills that produce the sharpest pressure spike or the fastest repeated impact.

If your leaking shows up only on a specific lift, that is useful information rather than a reason to cut the lift. It tells you where your pelvic floor's capacity currently tops out.
The Truth About Breath Holding and the Valsalva Maneuver
Many lifters are told that breath holding during a heavy lift is dangerous or wrong. You might have heard that you should always exhale on the way up.
Holding your breath, often called the valsalva maneuver, is a completely natural, normal, and effective way to lift heavy objects. When you take a deep breath and hold it while tightening your core, you create massive internal stability. It anchors your torso and actually helps you move more weight efficiently.
Is breath holding bad when lifting? No. It is a tool. Because it creates a massive spike in internal pressure, it places a heavy demand on your pelvic floor. If your pelvic floor cannot match that sudden downward pressure, the bladder valve yields.
Should You Hold Your Breath or Breathe Through It?
Because there is no single correct or safe way to breathe, your lifting technique should adapt based on what your body is experiencing. You can decide whether to hold your breath or modify your breathing by tracking your symptoms using the guidelines below.

Run this check set by set rather than deciding once. Your capacity changes with fatigue, load, sleep, and where you are in your cycle. The same weight that felt controlled in week one may push past your threshold in week four.
Urine Leakage During Exercise Outside the Weight Room
Barbells get the attention, but urine leakage during exercise shows up just as often in endurance training. Running generates a continuous stream of impact through the pelvic floor, roughly 1,500 to 1,800 foot strikes per mile, with no pause to reset between them. A pelvic floor that handles a single heavy squat may still fatigue and yield somewhere in mile three.
The retraining principle stays the same, though the demand differs. Lifting asks your pelvic floor for a large, brief output. Running asks for a smaller output repeated thousands of times. How to Stop Leaking (And Where Kegels Fit In)
If you are dealing with leakage, the automatic advice from the internet is usually: Do more Kegels!
While squeezing the pelvic floor can help, classic Kegels are only one piece of a much larger puzzle. A heavy squat requires your whole body to work together. Doing isolated Kegels while sitting in your car doesn't teach your pelvic floor how to automatically snap into action when a 150-pound barbell is on your back.
To stop leaking, your pelvic floor needs coordination, endurance, and strength that matches your actual workouts.
There is a second reason the Kegel-only advice backfires. Some athletes leak because their pelvic floor is already gripping too hard and cannot fully relax between efforts. A muscle stuck in a shortened position cannot generate force on demand. Adding more squeezing to a hypertonic pelvic floor makes the symptom worse, not better. Determining which pattern you have requires an internal assessment, not a guess.
What Retraining Actually Involves
Effective programming for exercise induced urinary incontinence works on four fronts:
Coordination. Teaching the pelvic floor to fire in time with the diaphragm rather than a beat behind it.
Endurance. Building the capacity to hold that closure through a full set or a full mile.
Load matching. Progressively rehearsing the contraction against the weights you actually train with.
Breath timing. Choosing the valsalva maneuver or a slow exhale based on the lift in front of you.
Most athletes see change within a few weeks once the work targets the right deficit.
Temporary Support with Pessaries
While you work on building long-term pelvic floor strength, you do not have to stop lifting. Pessaries are small, flexible silicone devices inserted into the vagina, similar to a tampon. They act as a mechanical support shelf for your urethra and bladder neck. A pessary is a fantastic temporary treatment option to keep you dry and confident in the gym while you train your muscles for permanent change.
Continence pessaries appear as a first-line conservative option in the American Urological Association's stress incontinence guideline, alongside pelvic floor muscle training. Using one is not giving up on the muscle work. It buys you training time while the muscle work takes hold.
